Hamana, S., V. Augusto, X. Xie. A Timed Petri Net Approach for Verification of Territorial Healthcare Information Systems. In Automation Science and Engineering (CASE), 2016 IEEE International Conference.

This paper addresses the performance evaluation and verification of territorial healthcare information systems. For this purpose, we propose a new class of timed Petri nets, called THIS nets (Territorial Healthcare Information Systems), which formally describe patient care-pathways, relevant information flows and their interactions. THIS nets are then used for verification of the health information systems and evaluation of performances such as cycle time distribution and probability of information availability at some target time. A real example of cancer patient healthcare information system is used to illustrate the usefulness of the proposed approach.

Résumé : Nous avons pu établir que l’évaluation d’un système d’information au domicile du patient passait par la prise en compte de sa valeur d’usage, ce qui induisait la prise en compte des rationalités des acteurs et du contexte de l’HAD. Nous avons montré comment, à partir d’une enquête qualitative portant sur deux HAD, nous pouvions rendre compte des usages du SI au domicile en faisant émerger les processus ainsi que les attentes des professionnels, usagers du SI, à partir d’entretiens.

A partir de ces éléments, nous avons pu pointer que les objectifs du SI n’était pas synonyme avec les attentes des professionnels et que ceux-ci étaient susceptibles d’opter pour des solutions alternatives, parfois informelles, susceptibles de faciliter la conduite de leurs objectifs. Cette tendance au contournement appelle à décoreller l’usage du SI et la satisfaction quant aux échanges d’information lors d’une évaluation d’un SI. A partir de ce constat, nous proposons de faire usage du modèle Delone et McLean en mettant au centre de l’évaluation la relation entre usage et satisfaction pour la qualité du SI, la qualité du support et le transfert d’informations entre les acteurs de la prise en charge du patient au domicile et la structure HAD.

COST ANALYSIS AND PERFORMANCE EVALUATION OF HEALTH INFORMATION SYSTEMS THROUGH MICROCOSTING AND DISCRETE-EVENT SIMULATION

Objective

Development and implementation of Information and Communication Technology (ICT) are required to improve patient care. In this study we propose a cost evaluation of hospital medical consultation taking into account the level of deployment and the integration of Health Information Systems (HIS) along with a performance evaluation approach to evaluate HIS implementation scenarios considering both quantitative and qualitative performance indicators.

Methods

Micro costing techniques have been used to calculate direct costs related to medical equipment and human resources with the hospital point of view. Data has been collected through observations and hospital surveys. Sensibility analyses have been conducted on the main cost vectors. Discrete-event simulation (DES) has been used to model and simulate scenarios to determine the elements of the HIS providing the best improvements.

Results

The study has been conducted on 179 patients from three hospitals with various level of HIS. The relative cost of the HIS is low (3 to 6 % of total cost). However, the usage rate of computers is high when the HIS is well deployed and integrated (30 to 40 % of total duration). A small reduction of consultation duration thanks to HIS allows a drastic reduction of the total cost. DES shows that the implementation of numeric patient file, voice recognition report dictation, and HIS integration are the best improvements to reduce costs.

Conclusions

The cost analysis coupled with a performance evaluation study allows to conclude on the impact of HIS on the consultation and it is possible to point out the best actions to take to improve the overall performance of the consultation. However the consultation duration depends on factors which was not taken into account, such as doctors’ experience or global organization of hospitals. Such parameters will be taken into account in the future to generalize the proposed results.
